ABSTRACT

Abstract We describe a case of unusual development of the celiac trunk observed in the cadaver of 1-year old male child. The celiac trunk branched into five vessels: the splenic, common hepatic and left gastric arteries, the left inferior diaphragmatic artery, and a short trunk that branched into the right inferior diaphragmatic artery and right accessory hepatic artery. Additionally, the manner of branching of the vessel was unusual: it was possible to distinguish two branching points that corresponded to its s-shaped trajectory. There were also other variations of vascular supply, such as the presence of a left accessory hepatic artery, an additional superior pancreatoduodenal artery, and others. It should be noted that multiple developmental variations can be common in clinical practice and clinicians should be aware of them during diagnostic and interventional procedures.


Resumo Apresentamos um relato de caso de desenvolvimento incomum do tronco celíaco em um cadáver do sexo masculino de 1 ano de idade. O tronco celíaco ramificou-se para cinco vasos: as artérias esplênica, hepática comum e gástrica esquerda, a artéria diafragmática inferior esquerda e um tronco pequeno que se ramificou para a artéria diafragmática inferior direita e para a artéria hepática direita acessória. Além disso, a forma como o vaso se ramificou foi incomum: é possível distinguir dois pontos de ramificação que correspondem à trajetória em formato de S. Também houve outras variações do suprimento vascular, como a presença da artéria hepática esquerda acessória, da artéria pancreaticoduodenal superior acessória e outras. Cabe observar que a variação de desenvolvimento múltipla pode ser comum na prática clínica, e os médicos devem estar cientes dela durante os procedimentos de diagnóstico e intervenção.

Abstract Background Aneurysms and atheromatous processes are prominent pathological features that are commonly associated with significant morbidity and mortality. Objectives This cadaveric study was conducted to evaluate the morphometric and histological aspects of atheromatous plaque formation in abdominal aortas and their branches and their associated morphological variations, if present, characterized by loops, kinking, or tortuosity. Methods The study was performed using 30 human cadavers (approx. 65-75 years). Frequency of occurrence of calcified plaques in the abdominal aorta and its branches and their morphometric measurements were noted and histological features were observed with the aid of Hematoxylin & Eosin staining. Results Variations in the abdominal aorta and the common iliac artery were observed in 16.6% of specimens. Atheromatous plaque formation was seen in 2 specimens (1 specimen was associated with kinking) while in 3 other specimens only variation in normal structure (kinking/ tortuous artery) was observed. Histological analysis showed foamy macrophages and dense calcification, giving an atheromatous appearance. Conclusions Cadaveric reports of the location, nature, and degree of plaque formation in the abdominal aorta and its branches are extremely important in clinical settings and for choosing treatment options.


Resumo Contexto Aneurismas e processos ateromatosos são características patológicas perceptíveis que costumam estar associadas a morbidade e mortalidade significativas. Objetivos Este estudo em cadáveres teve como objetivo avaliar os aspectos morfométricos e histológicos da formação de placa ateromatosa na aorta abdominal e em seus ramos com suas subsequentes variações morfológicas caracterizadas por enrolamentos, acotovelamentos ou tortuosidades, se presentes. Métodos O estudo foi realizado com 30 cadáveres humanos (aproximadamente 65-75 anos). Foi descrita a frequência de ocorrência de placas calcificadas na aorta abdominal e em seus ramos com as respectivas medidas morfométricas e características histológicas com auxílio da coloração por hematoxilina e eosina. Resultados Variações na aorta abdominal e na artéria ilíaca comum foram observadas em 16,6% dos espécimes. A formação de placa ateromatosa foi observada em dois espécimes (um espécime foi associado a acotovelamento), enquanto em outros três espécimes houve apenas variação na estrutura normal (acotovelamento/artéria tortuosa). A análise histológica mostrou macrófagos espumosos e calcificação densa, o que gerou uma impressão ateromatosa. Conclusões Os estudos em cadáveres sobre localização, natureza e grau de formação de placa na aorta abdominal e em seus ramos são extremamente importantes para os cenários clínicos e as opções de tratamento.

ABSTRACT

O interesse pelo estudo de animais silvestres vem crescendo consideravelmente nos últimos anos, seja em decorrência do risco de extinção ou visando o controle de doenças, especialmente as zoonoses. A ordem Rodentia apresenta o maior número de espécies da classe Mammalia. Apesar de ampla distribuição e importância, dados sobre sua anatomia vascular renal são escassos na literatura. O objetivo deste artigo é relatar o aparecimento de variação numérica na artéria renal esquerda em Sphiggurusvillosus com enfoque nas possibilidades de implicações clínico-cirúrgicas, como, anastomoses cirúrgicas, estudos imaginológicos, nefrectomias e planejamento pré-operatório para redução de riscos e complicações como hemorragia. O cadáver foi devidamente formolizado no Laboratório de Ensino e Pesquisa em Morfologia de Animais Domésticos e Selvagens e posteriormente dissecado. O rim esquerdo apresentou três artérias renais, uma cranial, uma intermediaria e outra caudal, ambas posicionadas em nível de L2 emergindo de forma impar lateralmente da aorta abdominal. A primeira artéria, mais cranial, apresentou 10,52 mm de comprimento e se dirigiu diretamente para o hilo renal, emitindo ramo para adrenal, diafragma e musculatura sublombar. A segunda artéria, intermediária, mediu 7,77 mm, emitiu ramo cranial e caudal para o hilo renal e ramo ureteral. A terceira artéria, caudal, mediu 10,11 mm e se dirigiu para o hilo renal. A veia renal esquerda era única e apresentou 9,25 mm de comprimento, posicionada em nível de L1. Este é o primeiro relato de artéria renal tripla em mamífero silvestre.


Interest in the study of wild animals has grown considerably in recent years, either due to the risk of extinction or to control diseases, especially zoonoses. The order Rodentia has the largest number of species in the Mammalia class. Despite its wide distribution and importance, data on its renal vascular anatomy are scarce in the literature. The aim of this paper is to report the appearance of numerical variation in the left renal artery in porcupine focusing on the possibilities of clinical and surgical implications, such as surgical anastomoses, imaging studies, nephrectomies and preoperative planning to reduce risks and complications such as bleeding. The animal was duly formalized in the Laboratory of Teaching and Research in Morphology of Domestic and Wild Animals and subsequently dissected. The left kidney had three renal arteries, one cranial, one intermediate, and one caudal, both positioned at L2 level, emerging unevenly laterally from the abdominal aorta. The first more cranial artery was 10.52 mm long and directed directly into the renal hilum, emitting a branch to the adrenal, diaphragm and sub lumbar muscles. The second intermediate artery measured 7.77 mm, emitting a cranial and caudal branch to the renal hilum and ureteral branch. The third caudal artery measured 10.11 mm and headed straight for the renal hilum. The left renal vein was unique, measured 9.25 mm long, and positioned at L1 level. This is the first report of triple renal artery in wild mammals.

Resumo Contexto Não se sabe ao certo como a idade e o sexo do paciente influenciam na anatomia da aorta abdominal e de seus ramos. Objetivos Determinar os padrões anatômicos (diâmetro e angulações) mais frequentes da aorta abdominal e de seus ramos e a influência do sexo e da idade dos pacientes sobre esses padrões. Métodos Foram avaliadas tomografias computadorizadas de abdome com contraste endovenoso de 157 pacientes. Foram aferidos calibre e angulação de artérias abdominais em indivíduos de ambos os sexos, agrupados em cinco faixas etárias: 20 a 30 anos, 31 a 40 anos, 41 a 50 anos, 51 a 60 anos e 61 a 70 anos. Foram analisadas 18 variáveis: seis ângulos de emergências arteriais, nove diâmetros arteriais, taxas de dilatação, sexo e faixa etária. Para a obtenção das medidas, utilizou-se o programa de computador RadiAnt 4.2.1 DICOM viewer (Medixant, Poznan, Polônia). Resultados Entre as 157 tomografias, 69 eram de homens e 88, de mulheres. Apresentaram diferença estatística (p < 0,05): ângulo de origem e diâmetro da artéria mesentérica superior; ângulo e diâmetro das artérias renais; diâmetro das artérias ilíacas comuns; diâmetro e taxa de dilatação em diversos segmentos da aorta, exceto na porção proximal ao tronco celíaco. Conclusões Os diâmetros da aorta (em diversos segmentos) e de seus ramos (exceto da artéria renal esquerda) aumentam progressivamente com o passar da idade em ambos os sexos e são maiores e possuem taxa de dilatação mais elevada em homens do que em mulheres da mesma faixa etária. Entre os sexos, o ângulo de emergência da artéria mesentérica superior foi maior em homens, exceto entre 20 e 30 anos; o ângulo de origem da artéria renal esquerda foi maior em mulheres entre 51 e 60 anos.


Abstract Background It is not clear how patients' age and sex influence the anatomy of the aorta and its branches. Objectives To determine the most frequent anatomical patterns of diameter and angulation of the abdominal aorta and its branches and the influence of patients' sex and age on these patterns. Methods CT scans with intravenous contrast from 157 patients were analyzed. Diameter and angulations of the abdominal aorta and its branches were measured in individuals of both sexes, classified into five age groups: 20 to 30 years, 31 to 40 years, 41 to 50 years, 51 to 60 years, and 61 to 70 years. Eighteen variables were analyzed: 6 arterial origin angles, 9 arterial diameters, rate of diameter enlargement, and patient's sex and age. RadiAnt 4.2.1 DICOM viewer software was used for measurements. Results The total of 157 CT scans were from 69 men and 88 women. There were statistical differences (p <0.05) in the following results: angle of origin and diameter of the superior mesenteric artery; renal artery angle and diameter; diameter of the common iliac arteries, and diameter and rate of diameter enlargement of the aorta in several segments, but not the segment immediately proximal to the celiac trunk. Conclusions The diameters of several aorta segments and of its branches (except the left renal artery) increase progressively with age in both sexes and are larger and have a higher rate of diameter enlargement in men than in women in the same age ranges. Between sexes, the angle of origin of the superior mesenteric artery was larger in men, except between 20 and 30 years, and the angle of origin of the left renal artery was larger in women between 51 and 60 years old.

ABSTRACT

ABSTRACT Endovascular aneurysm repair is an established technique for treating many infrarenal aortic aneurysms. Infection is one of the most serious complications of this technique, and although percutaneous treatment has been well established for intra-abdominal collections, its use to treat peri-prosthetic fluid collections has not been well determined. In this article we describe a small series of three patients who were treated with percutaneous drainage, with good clinical and imaging responses. Percutaneous drainage is a safe, effective and minimally invasive approach for treating this potentially fatal complication.


RESUMO O reparo endovascular de aneurisma é uma técnica para tratamento de diversos aneurismas infrarrenais da aorta. A infecção é uma das complicações mais sérias desse tratamento, e a abordagem percutânea tem sido adotada para coleções intra-abdominais, mas seu uso para tratamento de coleções protéticas vasculares não está bem estabelecido. Descreve-se, neste trabalho, pequena série de três pacientes tratados com drenagem percutânea, que apresentaram boa resposta clínica e de imagem. A drenagem percutânea mostrou-se abordagem segura, efetiva e minimamente invasiva para tratamento desta complicação, que é potencialmente fatal.

ABSTRACT

The hoary fox (Lycalopex vetulus,Lund, 1842) is the smallest Brazilian canid, whose weight varies between 2 and 4 kg, has a slender body, a small head, and a short and blackened snout. Despite being considered an endemic species, little is known about the hoary fox as it is one of the seven less studied canids in the world. Thus, this study aimed to describe the anatomy of the abdominal aorta artery of the hoary fox and to compare it with the pre-established literature data in domestic canids. For this purpose, we used two adult hoary foxes without definite age. We collected the corpses of these animals along roadsides of Catalão-GO, being later fixed and conserved in a 10% formalin solution. The results showed that the abdominal aorta in hoary fox is at the ventral face of the lumbar region vertebral bodies, being slightly displaced to the left of the median plane. The first branch is visceral, named celiac artery, followed by a paired parietal branch: the phrenic abdominal arteries. The third and fourth branches are the cranial mesenteric arteries and the right and left are the renal arteries, respectively. The posterior branches of the renal arteries are equally visceral, paired, being called testicular arteries. Distal to the latter, both the caudal mesenteric artery and deep circumflex iliac arteries originate. Finally, two large external iliac arteries and its terminal branches composed of internal iliac arteries and the median sacral artery originate. In addition, five pairs of lumbar arteries originate alongside the abdominal segment of the aorta. Considering these findings, it is possible to conclude that despite the hoary fox being a wild animal, the anatomy of its abdominal aorta is very similar to that of domestic canids.(AU)


A raposa-do-campo (Lycalopex vetulus,Lund, 1842) é o menor canídeo brasileiro, cujo peso varia entre 2 e 4 quilos, possui corpo esguio, a cabeça é pequena, focinho curto e enegrecido. Considerada uma espécie endêmica, pouco se sabe a seu respeito, e é um dos sete canídeos menos estudados no mundo. Assim, o presente estudo teve o objetivo de descrever a anatomia da parte abdominal da artéria aorta em raposa-do-campo e comparar com dados literários pré-estabelecidos de canídeos domésticos. Para a realização deste estudo foram utilizados dois exemplares de raposa-do-campo, adultos, sem idade definida. Os cadáveres dos animais foram recolhidos às margens de rodovias no entorno da Catalão-Goiás, fixados em solução aquosa de formol a 10% e conservados na mesma solução. Os resultados mostraram que a aorta abdominal da raposa-do-campo está localizada sobre a face ventral dos corpos vertebrais da região lombar, levemente deslocada para a esquerda do plano mediano. O primeiro ramo é visceral, denominado artéria celíaca, seguido por um ramo parietal, pareado, as artérias frênico-abdominais. O terceiro e quarto ramos são a artéria mesentérica caudal e as artérias renais direita e esquerda, respectivamente. Os ramos posteriores das artérias renais são igualmente viscerais, pareados, denominados artérias testiculares. Distal à essas últimas, originam-se a artéria mesentérica caudal e as artérias circunflexas ilíacas profundas. Finalmente surgem duas grandes artérias ilíacas externas e os ramos terminais compostos pelas artérias ilíacas internas e artéria sacral mediana. Ao longo do trajeto da aorta abdominal, cinco pares de artérias lombares se originam da face dorsal. Considerando esses achados, pode ser concluído que a anatomia da aorta abdominal da raposa-do-campo é muito similar àquela de canídeos domésticos, embora ela seja um animal silvestre. (AU)

ABSTRACT

The abdominal aorta of the rabbit has been in the focus of research to develop new platforms of training diagnostic and therapeutic protocols; and for testing endovascular devices and materials, however, few descriptions of the anatomy of the abdominal aorta and its emerging visceral branches has been reported on the scientific literature for this specie. Anatomical variations are common and should have in mind during research and clinical trials. The aim of this study was to describe the different patterns that can occur in the visceral branches arising from the abdominal aorta in the rabbit.


La aorta abdominal del conejo ha sido objeto de estudio e investigación para desarrollar nuevas plataformas de entrenamiento para protocolos diagnósticos y terapéuticos así como para probar las virtudes de materiales y equipos endovasculares, sin embargo, existen muy pocas descripciones en la literatura de las ramas viscerales que emergen de la aorta abdominal en esta especie. Las variaciones anatómicas son eventos de ocurrencia común y por lo tanto deben ser considerados para realizar investigación y ensayos clínicos. El objetivo del presente estudio ha sido describir los diferentes patrones que pueden presentar las ramas viscerales que emergen de la aorta abdominal en el conejo.

Abstract Background Knowledge of testicular artery variations is vital to ensure that they are not neglected during a variety of different operative techniques, since damage can cause testicular atrophy. Objectives The present study was therefore intended to identify variants in the origin and course of the testicular arteries. An attempt was made to classify the arteries based on their various origins. Methods This study examined 42 formalin-fixed cadavers of 40 to 70-year-old adult males. Variant origins of the testicular artery were identified and classified. Variations in the origin and course of the artery were colored, photographed, and documented. The distances between the origins of the testicular arteries and the mid-points of the origins of the renal arteries were measured. Results Testicular arteries were classified into four categories on the basis of origin. This variability was defined in relation to the renal and inferior mesenteric arteries. The mean distance between the origin of the testicular artery and the mid-point of the origin of the renal artery were 3.08 and 3.47 cm, on the right and left sides respectively. Variations were almost exclusively found on the left side. The variations observed included multiple arterial twigs forming the testicular artery, suprarenal arteries arising from the testicular artery, and testicular artery duplication. Conclusion This study provides an insight into variations in the testicular artery and proposes a classification which could help surgeons during a variety of procedures on the male abdomen and pelvis.


Resumo Contexto O conhecimento de variações da artéria testicular é vital para assegurar que essas artérias não serão negligenciadas durante a realização de diferentes técnicas operatórias, já que qualquer dano poderia causar atrofia testicular. Objetivos Este estudo teve como objetivo identificar variações na origem e no trajeto das artérias testiculares. Foi feita uma tentativa de classificar as artérias com base em suas diferentes origens. Métodos Este estudo examinou 42 cadáveres de adultos do sexo masculino com idade entre 40 e 70 anos preservados em formol. As diferentes origens da artéria testicular foram identificadas e classificadas. Variações na origem e no trajeto da artérias foram coloridas, fotografadas e documentadas. Foram medidas as distâncias entre a origem das artérias testiculares e o ponto médio da origem das artérias renais. Resultados As artérias testiculares foram classificadas em quatro categorias com base em sua origem. Essa variabilidade foi definida com relação às artérias renal e mesentérica inferior. A distância média entre a origem da artéria testicular e o ponto médio da origem da artéria renal foi de 3,08 e 3,47 cm nos lados direito e esquerdo, respectivamente. As variações foram encontradas quase exclusivamente no lado esquerdo. As variações observadas incluíram múltiplos ramúsculos formando a artéria testicular, artérias suprarrenais surgindo a partir da artéria testicular, e duplicação da artéria testicular. Conclusão Este estudo traz informações sobre variações da artéria testicular e propõe uma classificação que poderia ajudar os cirurgiões durante diferentes procedimentos realizados no abdome e na pelve de pacientes do sexo masculino.

ABSTRACT

El diámetro de la parte abdominal de la aorta (AAI), constituye un factor relevante para decidir reparar un aneurisma aórtico abdominal. El objetivo de este estudio es determinar rangos de normalidad de los diámetros de la AAI, medidos con ultrasonografía 2D, en sujetos sin antecedentes de enfermedad vascular. Estudio de corte transversal, realizado en los Hospitales Regional de Temuco y de Pitrufquen. Se estudiaron 399 sujetos mayores de 15 años, sin antecedentes de enfermedad vascular, que consultaron por cuadros de dolor abdominal. Se realizó ultrasonografía 2D con dos equipos diferentes (General Electric RT3200 Advantage II y Medison co. Ltda. Modelo SA-600; ambos con transductores de 3,5 Mhz.) y por dos observadores independientes. Las variables resultado fueron diámetros antero-posterior (DAP) y transverso (DT) de la AA. Otras variables de interés fueron: sexo, edad índice peso/talla y superficie corporal. Se aplicó estadística descriptiva, inferencial aplicando t test para variables continuas, x2 para variables categóricas, ANOVA y Kruskal Wallis para comparación de grupos; estudios de correlación y una regresión lineal simple. El DAP promedio fue de 16,1 ± 2,2 mm (9 a 23 mm) y el DT promedio fue de 19,4 ±2,7 mm (11 a 26 mm). Se observaron DAP y DT significativamente mayores en el subgrupo de >50 años y en hombres. Hubo correlación positiva entre índice peso/talla-DAP (p=0,0321) e índice peso/talla-DT (p=0,0052), más relevante en el subgrupo femenino. Por otra parte, se demostró correlación positiva entre DAP y superficie corporal (p<0.0001) y DT y superficie corporal (p<0.0001). Sexo, edad y superficie corporal se asocian con DAP y DT más elevados. Al parecer la población estudiada posee AAI de menor diámetro promedio que lo descrito en la literatura internacional.


The diameter of the abdominal aorta (AA), is a relevant factor to repair an abdominal aortic aneurysm. The aim of this study is to determine the normal range of the diameter of the infrarenal AA according 2D ultrasonography in patients with no history of vascular disease. Cross-sectional study, conducted in Hospital Regional of Temuco and Pitrufquen in 399 subjects over 15 years, with no history of vascular disease, who consulted for abdominal pain cadres were studied. 2D ultrasonography was performed with two different computers (General Electric RT3200 Advantage II and co Medison. Ltda. Model SA-600, both with 3.5 MHz transducers.) and two independent observers. The outcome variables were anteroposterior diameters (DAP) and transverse (TD) of AA. Other variables of interest were: sex, age index weight / height and body surface. Descriptive statistics were applied, applying inferential t test for continuous variables, x2 for categorical variables and Kruskal Wallis ANOVA for comparison of groups; correlation studies and linear regression. DAP average was 16.1 ± 2.2 mm (9-23 mm) and DT average was 19.4 ± 2.7 mm (11-26 mm). DAP and DT was significantly higher in the subgroup age > 50 and in men. There was a positive correlation between weight / height-DAP (p=0.0321) and index weight / height-DT (p=0.0052), the more relevant in the female subgroup index. Moreover, positive correlation between DAP and body surface area (p<0.0001) and DT and body surface area (p<0.0001) was demonstrated. Sex, age and body surface area are associated with higher DAP and DT. Apparently our population has lower average diameter aortas that described in the international literature.

ABSTRACT

Vascular structures are in greater danger during lumbar surgery. The purpose of this study is to describe the morphology of the inferior vena cava (IVC) related to the lumbar vertebra and aortic bifurcation (AB) and assessing the role of demographic values in these relations to decrease the risk of complications in the surgical interventions. The study was performed on Multidetector Computed Tomography (MDCT) images of 100 male and 100 female cases with an age range from 50 to 84 years. The morphometric values of the IVC obtained from the coronal, sagittal and the axial reformatted images were measured and compared with the demographic values. The distance from the IVC to the 1st lumbar vertebra (L1) and 2nd lumbar vertebra (L2) were measured as (26.5 mm and 18.1 mm) in males and (21.1 mm and 14.2 mm) in females with a high level of significance between genders; the distance from the IVC to the iliocaval confluence (IC), 3rd lumbar vertebra (L3) and 4rd lumbar vertebra (L4) were measured as (6.5 mm, 10.5 mm and 6.9 mm) in males and (4.9 mm, 9.1 mm and 5.5 mm) in females with significance between genders. The level of the IC was detected 46 % of males, 39 % of females at the level of lower third of L4. The vertical distance between the IC and the AB was measured and negative correlation between genders based on age was detected. Demographic values are important to consider the relationship of the IVC, the lumbar vertebra and the AB. The IVC was located further from the lumbar vertebra in males compared to females. Age increase played role in the approaching of the AB and the IC to each other in both gender and the IC to the promontory level in males.


Las estructuras vasculares corren gran peligro durante la cirugía lumbar. El propósito de este estudio fue describir la morfología de la vena cava inferior (VCI) en relación con las vértebras lumbares y la bifurcación aórtica (BA), junto con evaluar los valores demográficos de estas relaciones para disminuir el riesgo de complicaciones en las intervenciones quirúrgicas. Se utilizaron tomografías computadorizadas multidetector (TCMD) de 100 casos de hombres y mujeres entre 50 a 84 años de edad. Los valores morfométricos de la VCI se obtuvieron desde imágenes coronales y sagitales reformateadas, medidas y comparadas con los valores demográficos. Se midió la distancia de la VCI a la 1a (L1) y 2a vértebra lumbar (L2), en hombres de 26,5 mm y 18,1 mm respectivamente, y en mujeres de 21,1 mm y 14,2 mm, respectivamente, con un alto nivel de significancia entre el sexos. La distancia desde la VCI a la confluencia iliocava (CI), 3a (L3) y 4a vértebra lumbar (L4) fue para los hombres de 6,5 mm, 10,5 mm y 6,9 mm, respectivamente, y en mujeres de 4,9 mm, 9,1 mm y 5,5 mm respectivamente, con significancia entre los sexos. El nivel de la IC se detectó en 46 % de los varones y en el 39 % de las mujeres, a nivel de tercio inferior de L4. La distancia vertical entre la CI y la BA se tuvo una correlación negativa entre sexos en base a la edad. Los valores demográficos son importantes al considerar la relación de la VCI, las vértebras lumbares y la BA. La VCI se encuentra a una distancia mayor de las vértebras lumbares en hombres que en mujeres. El incremento de la edad jugó un papel importante en la aproximación de la BA y la IC, tanto entre sí como según sexo, con la CI a nivel del promontorio en los hombres.

Abstract Background: Configuration of the abdominal aorta is related to healthy aging and a variety of disorders. Objectives: We aimed to assess heritable and environmental effects on the abdominal aortic diameter. Methods: 114 adult (69 monozygotic, 45 same-sex dizygotic) twin pairs (mean age 43.6 ± 16.3 years) underwent abdominal ultrasound with Esaote MyLab 70X ultrasound machine to visualize the abdominal aorta below the level of the origin of the renal arteries and 1-3 cm above the bifurcation. Results: Age- and sex-adjusted heritability of the abdominal aortic diameter below the level of the origin of the renal arteries was 40% [95% confidence interval (CI), 14 to 67%] and 55% above the aortic bifurcation (95% CI, 45 to 70%). None of the aortic diameters showed common environmental effects, but unshared environmental effects were responsible for 60% and 45% of the traits, respectively. Conclusions: Our analysis documents the moderate heritability and its segment-specific difference of the abdominal aortic diameter. The moderate part of variance was explained by unshared environmental components, emphasizing the importance of lifestyle factors in primary prevention. Further studies in this field may guide future gene-mapping efforts and investigate specific lifestyle factors to prevent abdominal aortic dilatation and its complications.


Resumo Fundamento: A configuração da aorta abdominal relaciona-se com o envelhecimento saudável e uma série de distúrbios. Objetivos: Avaliar efeitos herdáveis e ambientais no diâmetro da aorta abdominal. Métodos: 114 pares de gêmeos adultos (69 monozigóticos e 45 dizigóticos do mesmo sexo), com idade média de 43,6 ± 16,3 anos, foram submetidos a ultrassonografia abdominal com o aparelho Esaote MyLab 70X para visualização da aorta abdominal abaixo da origem das artérias renais e 1-3 cm acima da bifurcação aórtica. Resultados: A herdabilidade ajustada para idade e sexo do diâmetro da aorta abdominal abaixo da origem das artérias renais foi 40% [intervalo de confiança (IC) 95%, 14 – 67%] e acima da bifurcação, 55% (IC 95%, 45 – 70%). Nenhum dos diâmetros aórticos apresentou efeitos ambientais comuns, mas os efeitos ambientais não compartilhados foram responsáveis por 60% e 45% dos traços, respectivamente. Conclusões: Nossa análise mostrou herdabilidade moderada e diferença do diâmetro da aorta abdominal com especificidade de segmento. A parte moderada da variância foi explicada pelo componente ambiental não compartilhado, enfatizando a importância do estilo de vida na prevenção primária. Estudos adicionais nesse campo poderão guiar futuros esforços de mapeamento genético e investigar fatores específicos de estilo de vida para prevenir dilatação da aorta abdominal e suas complicações.

ABSTRACT

A jaguatirica (Leopardus pardalis) é uma das espécies de felino silvestre que pouco foi investigada quanto a sua morfologia. Assim, o estudo objetivou detalhar a origem e distribuição dos ramos colaterais da aorta abdominal deste animal. [...] A aorta abdominal do L. pardalis teve origem entre T12 e L1, sendo a artéria celíaca o primeiro ramo visceral no sentido crânio-caudal, resultando nas artérias hepática, gástrica esquerda e esplênica. A artéria mesentérica cranial surgiu como segundo ramo da aorta abdominal, originando as artérias jejunais. Na sequência localizamos artéria pancreáticoduodenal caudal, artérias ileais, artérias ileocólicas, artérias renais direita e esquerda, artérias adrenais direita e esquerda e artérias ováricas ou testiculares direita e esquerda. Parietalmente, a aorta abdominal originou em média seis ramos lombares, bem como a artéria frenicoabdominal, as artérias circunflexas ilíacas profundas e artérias ilíacas externa e interna. A aorta abdominal gerou ainda a artéria mesentérica caudal, a qual dividiu-se em artérias cólica esquerda e retal cranial. A artéria cólica esquerda seguiu cranialmente paralela ao cólon descendente irrigando-o, originando em média 18 ramos, e anastomosando-se com a artéria cólica média. A artéria retal cranial seguiu em direção caudal distribuindo oito ramos à porção final do cólon descendente e ao reto, e uniu-se com a artéria retal média. Por fim, a aorta abdominal emitiu como ramo terminal a artéria sacral mediana. A vascularização arterial abdominal desta espécie é bastante semelhante ao descrito em felinos domésticos e demais mamíferos, com diferenças quanto ao número de artérias jejunais e origem das artérias renais.

The cranial rectal artery followed caudally emerging into eight branches uniting with the final portion of the descending colon and the rectum, together with the rectal middle a.. Finally, the abdominal aorta emerged as the terminal branch, the median sacral. The abdominal arterial vascularization of the ocelot is quite similar to that described in domestic cats, with differences in the number of jejunal arteries and origin of the renal arteries. The abdominal arterial vasculature of this species is quite similar to that described in domestic cats and other mammals, differing in number of the jejunal arteries and origin of the renal arteries.

OBJECTIVE: To evaluate the value of spinal and paraspinal anatomic markers in both the diagnosis of lumbosacral transitional vertebrae (LSTVs) and identification of vertebral levels on lumbar MRI. MATERIALS AND METHODS: Lumbar MRI from 1049 adult patients were studied. By comparing with the whole-spine localizer, the diagnostic errors in numbering vertebral segments on lumbar MRI were evaluated. The morphology of S1-2 disc, L5 and S1 body, and lumbar spinous processes (SPs) were evaluated by using sagittal MRI. The positions of right renal artery (RRA), superior mesenteric artery, aortic bifurcation (AB) and conus medullaris (CM) were described. RESULTS: The diagnostic error for evaluation of vertebral segmentation on lumbar MRI alone was 14.1%. In lumbarization, all patients revealed a well-formed S1-2 disc with squared S1 body. A rhombus-shaped L5 body in sacralization and a rectangular-shaped S1 body in lumbarization were found. The L3 had the longest SP. The most common sites of spinal and paraspinal structures were: RRA at L1 body (53.6%) and L1-2 disc (34.1%), superior mesenteric artery at L1 body (55.1%) and T12-L1 disc (31.6%), and AB at L4 body (71.1%). CM had variable locations, changing from the T12-L1 disc to L2 body. They were located at higher sacralization and lower lumbarization. CONCLUSION: The spinal morphologic features and locations of the spinal and paraspinal structures on lumbar MRI are not completely reliable for the diagnosis of LSTVs and identification on the vertebral levels.

ABSTRACT

PURPOSE: It is important to know the normal diameter of artery throughout the body so that clinicians are able to determine when an artery becomes aneurysmal. However, there are no previous studies on the normal diameter of arteries in the general Korean population. The purpose of this article is to determine the normal reference diameters of the abdominal aorta and iliac arteries in the Korean population. MATERIALS AND METHODS: We recruited the study population from three cities in Korea for the abdominal aortic aneurysm (AAA) screening. We measured the diameter of the aorta and iliac arteries. We analyzed the reference diameter of the population without AAA. The results were analyzed by Student's t-test and ANOVA on SPSS version 19. A p value <0.05 was considered to be statistically significant. RESULTS: One thousand two hundred and twenty-nine people were enrolled. 478 men and 751 women, with a mean age of 63.9+/-10.1 years (range 50 to 91) were examined. Eleven out of 1229 (0.89%) were diagnosed with AAA. In the population of 1218 people without AAA, the mean diameters (cm) of male/female were 2.20/2.11 (p<0.001) at suprarenal, 2.04/1.90 (p<0.001) at renal, 1.90/1.79 (p<0.001) at infrarenal, 1.22/1.17 (p<0.001) at right iliac and 1.47/1.15 (p=0.097) at the left iliac, respectively. There was a significantly larger diameter in the male population. The diameter of each level increased with age. CONCLUSION: The normal reference diameter of the infrarenal abdominal aorta in the Korean population is 1.9 cm in males and 1.79 cm in females. The diameter of the abdominal aorta increases with age.

ABSTRACT

The gonadal arteries (testicular or ovarian arteries) emerge normally from the lateral aspect of the abdominal aorta, a little inferior to the renal arteries. Several other sites of origin of these arteries have been recorded with the renal and accessory renal arteries being the most common. In the present case report, the testicular arteries originated from the lower polar accessory renal arteries in both sides. The testicular veins followed had the usual origin and course, while an accessory renal vein was observed only in the right side. These anomalies were combined with an abnormal left ureter exiting from the lower pole of the kidney. Only one male cadaver among 77 adult human cadavers of Caucasian origin presented this set of variations (frequency: 1.3%). Variations of renal and gonadal vessels are important, as their presence could result in vascular injury of any accessory or aberrant vessel if the surgeon does not identify them.


Las arterias gonadales (testiculares o ováricas) se originan normalmente de la parte lateral de la parte abdominal de la aorta, distal a las arterias renales. Se han registrado otros lugares de origen de estas arterias, entre ellos, los más comunes en las arterias renales y renales accesorias. En el presente caso, las arterias testiculares se originaron bilateralmente desde las arterias renales polares inferiores accesorias. Las venas testiculares siguieron el origen y curso habitual, mientras que una vena renal accesoria sólo se observó en el lado derecho. Estas anomalías se combinaron con un uréter izquierdo anormal que salía desde el polo inferior del riñón. Sólo un cadáver de sexo masculino, de origen caucásico, entre los 77 cadáveres humanos adultos, presentaba este conjunto de variaciones (frecuencia: 1,3%). Las variaciones de los vasos renales y gonadales son importantes. Su presencia, si el cirujano no los identifica, puede resultar en lesiones vasculares de algún vaso accesorio o aberrante.

ABSTRACT

Variations in the branches of the abdominal aorta were determined during a routine abdominal region dissection of a 70-year-old male cadaver. Left gastric artery arose as the first root from antero-lateral of aorta. Coeliacomesenteric trunk occurred as a thick root. After 29.9mm, coeliacomesenteric trunk bifurcated as coeliac trunk and superior mesenteric artery. Coeliac trunk bifurcated as splenic artery and common hepatic artery. These multiple variations which change the normal anatomic structure of the abdominal aorta have to be kept in mind by surgeons, radiologists and anatomists.


Fueron encontradas, en un cadáver de sexo masculino de 70 años de edad durante una disección de rutina de la cavidad abdominal, variaciones de las ramas en la parte abdominal de la aorta. La arteria gástrica izquierda se originaba como la la primera rama antero-lateral de la aorta. El tronco celiacomesénterico se originó desde la aorta como una raíz gruesa. Después de 29,9mm, el tronco celiacomesentérico se dividió en el tronco celíaco y la arteria mesentérica superior. El tronco celíaco se dividió en las arterias esplénica y hepática común. Estas variaciones múltiples que cambian la estructura anatómica normal de la parte abdominal de la aorta tienen que ser tomada en consideración por los cirujanos, radiólogos y anatomistas.

ABSTRACT

One of the locations of renal artery atherosclerosis is at the orifice of the renal artery, therefore the structure of this orifice was assessed in 6 normal, adult male dogs by light microscopy [LM] and transmission electron microscopy [TEM]. For the LM study, processed tissues were embedded in paraffin and sectioned serially into 6-?m thickness. Sections were stained with orcein. For the TEM study, after fixation and dehydration, samples were embedded in resin, and ultrathin sections stained with uranil acetate. In the LM study, two edges [proximal and distal] at the orifice of the renal artery were observed. Extra formation of elastin was clearly seen at the tip of the distal edge compared with the proximal edge. The curve from the aorta wall into the renal artery at the distal edge of the orifice was sharper than that of the proximal edge. In the TEM study, the tunica intima in the distal and lateral edges was thicker than that in the proximal edge. At the proximal edge of the orifice, there was only one layer of endothelial cells, and the internal elastic membrane was situated directly beneath it. In the distal edge, besides the endothelial cells, smooth muscle cells [SMCs] were found in the tunica intima that were separated from each other with elastic and collagen fibers. In the tunica intima of the lateral edge, two layers of SMCs were observed, between which were collagen fibers. This distance between the internal elastic membrane and endothelial cells at the distal and lateral edges can be important in diseases such as atherosclerosis. These data indicate that there are differences between the proximal edge and the distal and lateral edges of the orifice, and the involvement of special structures in atherosclerosis is suggested

ABSTRACT

PURPOSE: The present study was aimed to assess the feasibility of using decellularized aortic allograft in a rat small animal surgical model for conducting small diameter vascular tissue engineering research. MATERIALS AND METHODS: Decellularized aortic allografts were infra-renally implanted in 12 Sprague-Dawley (SD) adult rats. The conduits were harvested at 2 (n = 6) and 8 weeks (n = 6), and assessed by hematoxylin and eosin (H&E), van Gieson, Masson Trichrome staining, and immunohistochemistry for von Willebrand factor, CD 31+, and actin. RESULTS: Consistent, predictable, and reproducible results were produced by means of a standardized surgical procedure. All animals survived without major complications. Inflammatory immune reaction was minimal, and there was no evidence of aneurysmal degeneration or rupture of the decellularized vascular implants. However, the aortic wall appeared thinner and the elastic fibers in the medial layer showed decreased undulation compared to the normal aorta. There was also minimal cellular repopulation of the vascular media. The remodeling appeared progressive from 2 to 8 weeks with increased intimal thickening and accumulation of both collagen and cells staining for actin. Although the endothelial like cells appeared largely confluent at 8 weeks, they were not as concentrated in appearance as in the normal aorta. CONCLUSION: The results showed the present rat animal model using decellularized vascular allograft implants to be a potentially durable and effective experimental platform for conducting further research on small diameter vascular tissue engineering.

ABSTRACT

Durante el abordaje anterior de la región lumbosacra de la columna vertebral es necesario considerar la disposición de los vasos relacionados, evitando así cualquier lesión en ellos. La información sobre la bifurcación de la aorta referida en los textos de anatomía señalan que ésta se realiza a nivel de la cuarta vértebra lumbar. Con el propósito de identificar el nivel de bifurcación de la aorta, realizamos un estudio de registros angiotomográficos obtenidos a través de tomografia axial computarizada de 74 pacientes chilenos, adultos, de ambos sexos, 40 hombres y 34 mujeres. El nivel de bifurcación fue relacionado con los componentes de la columna vertebral, dividiendo las vértebras en tercios. En 10 pacientes (13,5 por ciento) la bifurcación se observó a nivel de la parte inferior de L3; en 17 (23 por ciento) en la parte superior de L4; en 8 (10,8 por ciento) en la parte media de L4; en 26 (35,1) en la parte inferior de L4; en 9 (12,2 por ciento) en la parte superior de L5 y en 4 (5,4 por ciento) en la parte inferior de L5.Los resultados obtenidos muestran que en la mayoría de los individuos estudiados la bifurcación de la aorta se produce a nivel de L4, existiendo menores porcentajes hacia la parte cefálica o caudal de la vértebra mencionada. Esta relación es un aporte al conocimiento anatómico de los vasos abdominales de nuestra población.


During the anterior approach of the lumbosacral spine is necessary considerate the relationships with the vascular system to prevent injuries. The classic anatomy texts describe the aortic bifurcation to level of fourth lumbar vertebra. The objective of this research was determine this level in Chilean individuals. We studied the aortic bifurcation by axial computed tomography in 74 patients, Chilean, adults, of both sexes, 40 men and 34 women. The mentioned level was related with the lumbar vertebras, dividing its body in third parts. We observed the bifurcation at level of inferior third of L3 in 10 patients (13.5 percent); in the superior third of L4 in 17 (23 percent); in the middle third of L4 in 8 (10.8 percent); in the inferior third of L4 in 26 (35.1 percent); in the superior third of L5 in 9 (12.2 percent) and in the inferior third of L5 in 4 (5.4 percent). The results shown that in 2/3 of the cases the aortic bifurcation was to L4 level, being less frequent in proximal or distal vertebras. These relationships are important for the anatomical knowledge of the abdominal vascular system of our people.

ABSTRACT

PURPOSE: Despite the fact that anatomical variations of the celiac trunk are well explored in the literature, information on these vessels diameters is scanty. The aims of the present study were to describe the arterial diameters of the celiac trunk and its main branches, and to investigate if these diameters are altered in those cases presenting anatomical variations of these vessels. METHODS: Twenty-one formalin fixed adult male cadavers were appropriately dissected for the celiac trunk identification and arterial diameter measurements. Arteries measured included the celiac trunk and its main branches (splenic artery, left gastric artery and common hepatic artery), as the proper hepatic artery, right gastric artery, the left and right hepatic arteries and the gastroduodenal artery. RESULTS: From the 21 cadavers, 6 presented anatomical variations of, at least, one of the above mentioned branches. The average arterial diameter comparisons between groups (normal and variable) clearly showed smaller diameters for variable vessels, but with no significant difference. CONCLUSION: Our data indicates the possibility of a diameter reduction of the celiac trunk main branches in the presence of anatomical variations. This should be taken into account on the selection for the liver transplantation donors.


OBJETIVO: Embora as variações arteriais do tronco celíaco e seus ramos estejam bem documentadas na literatura, o mesmo não ocorre com as descrições dos diâmetros dessas artérias. O presente estudo tem por objetivo descrever os diâmetros arteriais do tronco celíaco e seus ramos em indivíduos normais, bem como investigar se esses diâmetros se apresentam alterados em indivíduos com variação anatômica desses vasos. MÉTODOS: Utilizamos 21 cadáveres adultos, do sexo masculino, previamente fixados em formol a 10 por cento que foram dissecados apropriadamente para a exposição do tronco celíaco e seus ramos. Com o auxílio de um paquímetro digital, foram obtidos os diâmetros externos dos seguintes vasos: tronco celíaco, artéria hepática comum, artéria gástrica direita, artéria esplênica, artéria gástrica esquerda, artéria gastroduodenal, artéria hepática própria e artérias hepáticas direita e esquerda. RESULTADOS: Dos 21 cadáveres avaliados, 6 apresentaram variação anatômica de pelo menos um dos ramos acima mencionados. Ao compararmos os diâmetros arteriais dos vasos normais com os que apresentaram variação, os variáveis apresentaram diâmetros menores, sem, entretanto alcançar níveis de significância. CONCLUSÃO: Nossos dados apontam para a possibilidade de uma diferença de diâmetros entre artérias abdominais normais e variáveis, sendo esse dado de implicância clínica para os transplantes hepáticos de doadores vivos.
